The optional name and selected subject are used to prepare a WhatsApp message. The website code does not store these fields in a database, cookies or local storage. Continuing places the message in a link sent to WhatsApp. You must review and send it in the app for the team to receive it.

Health information

The form does not request medical records or a clinical history. Do not put sensitive health information in the name field. Ask the team how to share records securely.

External services

The website loads Google Fonts and links to WhatsApp, Google Maps, Rede D’Or and educational sources. Those services may process technical information such as your IP address under their own policies. The hosting platform may also process access and authentication logs.

Cookies and storage

The website code does not add advertising or analytics tools or store the form fields. Hosting features and external services follow their own settings.
